Welcome to ConcertoCare

ConcertoCare delivers whole-person complex care for individuals with or at risk for chronic illness. Complex care includes wrap-around care, chronic disease management, urgent care, and behavioral health support. Care is provided by an in-network group of doctors, nurses, and social workers who are here to help you and your family. You will keep your current doctors while receiving these services.

ConcertoCare is an extra layer of convenient, personalized in-home medical services meant to support your health and give you more time to focus on the people and things you love. ConcertoCare offer you:

Regular medical check-ups in your home (or another place that is convenient to you)
Urgent visits when you need care unexpectedly
24/7/365 phone availability, including evenings, weekends, and all holidays
Virtual visits using a laptop, tablet, or phone (our teams can come help you with your virtual visit)
Help with understanding and taking your medications
Mental health support
Help before or after a hospital stay
Support for your caregivers

ConcertoCare works with your existing doctors to make sure your care is seamless. That means you don't need to spend time making phone calls and sending paperwork back-and-forth to make sure everyone's on the same page about your medical needs – our teams do that for you.

How does ConcertoCare work with my regular doctors and care team?

Think of ConcertoCare as a “bridge” between you and your regular care team. You will continue seeing your current doctors. ConcertoCare will work closely with them to make sure you are getting the treatment and support you need.

Does my health plan cover ConcertoCare's services?

ConcertoCare is in your plan’s network, and their services are covered by your health plan benefits. ConcertoCare services are 100% covered by your insurance. There are no out-of-pocket costs to you or your family.

How do I get started?

A member of the ConcertoCare team will call you within the next few days to see if the program is a good fit. They will set up a virtual or in-home visit if you are interested. Then, you will talk one-on-one and create a care plan tailored for you.
